Service Description

This is a booking for both Services: LPA and AMD A Lasting Power of Attorney gives you (the Donee) legal powers to make decisions for your loved ones (the Donor) when they can no longer do so. The Donor must be at least 21 years of age with mental capacity. How to apply for an LPA? From 14 November 2022, under the provisions of the Mental Capacity Act 2021, all submissions of the LPA for registration must be done online via the new OPG Online system (OPGO). 1. Complete the LPA document online  - Login using Singpass and fill in the required information  2. Your Donee(s) accept the appointment - Login using Singpass 3. Make an appointment with us at any of our 1 Medical Clinics. 4. On the day of the appointment, please come down to the Clinic for certification and completion of the form online. 5. Your LPA is submitted to OPGO immediately for registration. 6. Donor and Donee will be notified of LPA status via SMS/email when LPA is accepted and registered. An Advance Medical Directive (AMD) is a legal document that you sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life.  How to make an AMD? 1. Download the AMD Form and fill it up accordingly.  2. Make an appointment with us at any of our 1 Medical Clinics. 3. On the day of the appointment, please come down to the Clinic. Remember to bring the AMD Form and IC for verification.  4. Mail the completed form in a sealed envelope by mail or by hand to: ​Registrar of Advance Medical Directives at Ministry of Health, SingaporeCollege of Medicine Building, 16 College Road, Singapore 169854 5. Your AMD will be valid only when it is registered with the Registrar of Advanced Medical Directives. The Registrar will send you an acknowledgement when your AMD has been registered.
