Lasting Power of Attorney (LPA)
A Lasting Power of Attorney gives you (the Donee) legal powers to make decisions for your loved ones (the Donor) when they can no longer do so. The Donor must be at least 21 years of age with mental capacity.

Benefits of an LPA

Enables a person to make a personal, considered choice of a trusted proxy decision maker, who is reliable and competent to act in his/her best interests should he/she lose mental capacity one day.

Alleviates the stress and difficulties faced by loved ones who need to apply for a Deputyship order, if the person loses mental capacity without an LPA in place.

This Donee can be appointed to act in / decide on matters related to:

Your personal welfare: decisions relating to healthcare, daily activities, where you should life

Your property & affairs: decisions on your financial matters, e.g., managing bank account and property

Both your personal welfare and property & affairs.

Choosing a Donee​

Your donee should be someone you trust.

Your donee must be at least 21 years old.

Thereafter, visit our clinic to get your LPA certified. Our certificate issuer price is $70 (exclusive of GST).
Advance Medical Directive (AMD)
An Advance Medical Directive (AMD) is a legal document that you sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life.

Making an AMD is a voluntary decision. It is a criminal offence for any person to force you to make one against your will

How to make an AMD?

Download the AMD Form here and fill it up accordingly.

Make an appointment with us at any of our 1 Medical Clinics.

Please bring the completed AMD Form and your IC for verification.

We will mail the completed form to the ​Registrar of Advance Medical Directives at Ministry of Health.

Your AMD will be valid only when it is registered with the Registrar of Advanced Medical Directives. The Registrar will send you an acknowledgement when your AMD has been registered. ​
